The NZAMD candidate entry forms are available below in Word format (click & save the files to your computer to complete the forms electronically).
PLEASE READ THE FOLLOWING INSTRUCTIONS FOR COMPLETING YOUR CANDIDATE ENTRY FORMS ELECTRONICALLY:
Teachers may wish to fill these forms in on their computer before printing off and posting to the office along with the Cover Summary Sheets and Entry Fees.
As the layout and colour of these forms does serve a specific purpose we would ask that you help us by……
- Retaining the integrity of the layout, that is - the width of the columns, and the size and number of lines on each page ( 30 ).
- Using the specific colour for each discipline when printing the forms. We would be happy for you to use the reverse of the coloured forms already sent out to you in your exam pack.
